For decades, we’ve built systems out of hard-coded rules and fixed flows. They evolved from paper forms but lost the flexibility that made paper work. Paper carried context: the margin notes, the scribbled “see me before approval”, the quiet intelligence passed between people. Our digital systems kept the boxes, but not the understanding.
These systems work when the world is predictable. When everything stays the same. But the moment there's an exception, an edge case, a judgment call, they crack.
That’s why every “automated” organization still relies on armies of people filling the gaps between the lines of code. People who know when to ignore the rule, when to step outside the process, and whose memory carries the context the system forgot.
Entire industries have formed just to paper over the cracks. Business process outsourcing and shared services don’t exist because humans love repetitive work, they exist because our systems can’t adapt to real complexity.
Over time, humans learned to compensate. We stitched together knowledge across emails, chat threads, spreadsheets, random files and post-it notes. We built organizations on institutional memory rather than institutional systems.
Now, the acceleration of artificial intelligence is exposing just how fragile that arrangement really is.
Like human teams, LLMs can reason through novel situations and articulate judgments. But when you plug them into systems built on rigid rules (systems that can’t record why a decision was made, can’t trace reasoning, and can’t learn from good decisions) you’re just creating a more sophisticated way to generate undocumented failures.
